Mail Order Contacts :
Call the toll free number provided in your Member's Area to place an order for replacement contact lenses, sunglasses, and safety glasses at wholesale prices plus S&H. Federal Law requires you or the doctor to mail or fax a copy of your prescription to the company before you can place an order. Once a valid doctor's prescription is received, you and your family may place orders as often as you wish prior to the expiration date of the prescription.

HOW TO ACCESS THIS BENEFIT: At Retail Pharmacies
Use participating retail pharmacies when you need a prescription on a short-term basis, such as an antibiotic to treat strep throat. Also consider using a participating retail pharmacy for a "starter" supply of maintenance medications you take on an ongoing basis, i.e., medications that you will be ordering regularly by mail. SUPPLY OF MEDICATION
Typically, you can obtain a 30-day supply at a participating pharmacy; however, depending on your prescription, larger quantities may apply.
ORDERING NEW PRESCRIPTIONS
Prescriptions may be filled at any of over 42,000 participating pharmacies. Simply show your member 10 card to the pharmacist, and you'll automatically receive the plan's low prices.
ORDERING REFILLS
Simply call or re-visit your participating pharmacy. Have your member ID number and your prescription number available for reference.
Due to state regulations, the pharmacy prescription drugs service is not available to residents of Tennessee.
Hearing Instruments/Care

HOW TO ACCESS THIS BENEFIT :
1) Call the toll free number provided in your Member's Area for a brochure to be mailed to you. You will be called in 7 to 10 days for a detailed case history of the hearing problem. 2) Testing is recommended by an American Speech and Hearing Association or other certified audiologists. If you live in an area without access to an American Speech and Hearing Association (ASHA) certified audiologist, a recommendation can be made. 3) Your hearing aid comes with a 12 month standard manufacturer's warranty. Modifications will be made without cost during warranty should a change in hearing levels require different circuitry.
Due to state regulations, the mail order hearing aid service is not available to residents of Colorado, Missouri or Texas.
