Once you're a member, you will be provided with a complete fee schedule with discounted fees for many common dental procedures. Other procedures discounted under the Signature Wellness Plan offer savings similar to the sample above. Discounts vary by state, so please refer to your membership kit or ask your participating provider for specific details. Plan Notes and Disclosures |
In Colorado and Maryland, Plan members are required to pay $1.00 for each oral exam and bitewing x-ray. In Washington members are required to pay a $10 fee for oral exams and bitewing x-rays.
