Notes and Disclaimers

Orthodontic services span a wide variety of problems that can be very limited for some people to very extensive for others. Orthodontic treatment may be used to correct crowded or crooked teeth, large spaces between teeth, bad bites, and skeletal disharmonies. Every treatment plan is unique to the individual’s situation and the usual and customary fee quoted may vary depending on the individual’s diagnosis. Consult with your orthodontic provider prior to beginning treatment for an estimate of the total fees that apply to your condition.   Fees may vary slightly by region.  

Discounts based off providers usual and customary fees. Discounted fees are listed for visits to participating Orthodontists. Any procedure not listed on the complete fee schedule is not discounted. Consult with your provider prior to beginning any treatment.

Payment for services received is due at the time treatment is performed. Please pay the Participating Orthodontist directly.

Exclusions from the OrthoCare Orthodontic Discount Program

• Additional costs due to frequently missed appointments.
• Cost for lost or broken appliances and retainers.
• Active treatment (from time of banding) longer than 24 months.
• Retention therapy/follow-up visits that extend beyond twelve (12) months.
• Changes in treatment plan or additional services necessitated by an accident or trauma.
• Personalized special appliances such as “clear” or “invisible” braces, colored brackets,Invisalign System and similar special braces. Discount will apply to the routine orthodonticappliance portion of services.
• Orthodontic services in process.
• Orthodontic treatment for the alteration of vertical face height, treatment of cleft palate ortreatment of temporomandibular joint dysfunction (TMD).
• Hospital costs, medical costs or Orthognathic surgery.
• Services that are performed by a dentists or orthodontic provider who is not an OrthoCareOrthodontist or for services that are not listed on the OrthoCare fee schedule.
• Any orthodontic services rendered after a member becomes ineligible due to dependent age(23 or over) or any other change in eligibility. Members must remain active members the entire duration of treatment to receive the OrthoCare discount. The discount will apply only for the portion of treatment rendered while eligible.
• This discount may not be used in conjunction with other orthodontic discounts, fee schedule plans or DHMO plans.
• Dental providers are independent contractors and solely responsible for all treatment provided. American Dental Professional Services, LLC makes not expressed or implied warranties regarding services received and disclaim all liability for any and all services performed my contracted providers and their associates.
• OrthoCare is not an insurance product, but a reduced fee dental program.
• Fees are subject to change without direct notice. Please contact your OrthoCare Orthodontist or ADPS Customer Services for questions regarding current fees.
