Plan Summary

The OrthoCare Orthodontic Discount Program is a stand-alone program for orthodontic care. When using a contracted OrthoCare Orthodontist, you could save 15% to 20% on the services performed. This results in meaningful savings when compared to the Usual and Customary (UCR) fees charged by Orthodontists.

All procedures listed under the OrthoCare Orthodontic Discount Program offer similar savings to the sample above. Orthodontic services span a wide variety of problems that can be very limited for some people to very extensive for others. Orthodontic treatment may be used to correct crowded or crooked teeth, large spaces between teeth, bad bites, and skeletal disharmonies. Every treatment plan is unique to the individual's situation and the usual and customary fee quoted may vary depending on the individual's diagnosis. The fees above are an example of typical charges for the procedures listed but wide variances will occur due to the broad range of services this spans. Consult with your orthodontic provider prior to beginning treatment for an estimate of the total fees that apply to your condition. Fees may vary slightly by region.

²Discounts based off providers usual and customary fees. Discounted fees are listed for visits to participating Orthodontists. Check the fee schedule for your region for the applicable fee in your region. Any procedure not listed on the complete fee schedule (provided in your Member's Area once you join) is not discounted. Consult with your provider prior to beginning any treatment.

Orthodontic services in process are excluded.

This program is based geographically by ZIP code in terms of dentist participation and location. Fee schedules and savings may vary by ZIP code and by Provider within a ZIP code.

Payment for services received is due at the time treatment is performed. Please pay the Participating Orthodontist directly. Dental Service fees are subject to change without direct notice.
