The Eclipse Dental Plan is a discount or "reduced-fee" dental plan. This plan is designed for health conscious consumers looking to maintain their oral health and minimize their dental care expenses.
General dentists who participate in the Eclipse Dental Plan have agreed to accept a discounted fee as payment in full for each service performed. These discounted fees are up to 60% less than what a dentist would normally charge. A sample table of Eclipse Dental Plan discounted fees can be found below.

Once you're a member, you will be provided with a complete fee schedule with discounted fees for nearly every dental procedure. All procedures discounted under the Eclipse Dental Plan offer similar savings to the sample above. If you are looking for a specific dental procedure that is not on this sample, please contact our Member Services Center toll-free at (888) 632-5353 with the ADA Code of the procedure and we can provide the discounted fee.

2 Discounted fees are listed for visits to a participating general dentist - this fee is guaranteed according to your fee schedule. Any procedure not listed on the complete fee schedule (provided in your members area once you join) will be discounted 20% off the provider's usual and customary fee. Consult with your provider prior to beginning any treatment. All family members must go to the same general dentist's office, provider must be selected at the time of joining. Lab fees are additional.

Eclipse Plan members who visit a participating specialist receive a flat 20% discount off specialists' usual and customary fees. This includes a 20% discount at:

Periodontists - gums
Oral Surgeons - wisdom tooth or major extractions

Pediatric Dentists - child treatment
Endodontist - extensive root canals

You are entitled to choose any participating dentist on the Eclipse plan. Dental offices are located throughout Utah and Arizona. The dental provider listings on DentalPlans.com are provided directly from each dental network or dental plan, who are responsible for its accuracy and completeness. These listings are updated on a monthly basis, and every effort has been made to ensure that only participating active providers are listed. We strongly recommend that you confirm that a provider is currently participating before you receive any services from that provider. Please see Member's Agreement for more details.
